The captain set the autopilot to hold 2,000 feet so the crew could troubleshoot in a holding pattern over the dark Everglades.<\/p><p>Somewhere in the fiddling, someone bumped the control column, and the autopilot&rsquo;s altitude hold quietly disengaged. The TriStar began a descent so gentle that no one felt it. A soft altitude-alert chime sounded near an empty crew station. Of the 176 people on board, 101 died, including all three flight crew. Yet 75 survived &mdash; many pulled from the mud and sawgrass by airboat-borne rescuers through the night. It turned the cockpit from a place ruled by a single captain&rsquo;s attention into a team that cross-checks itself.
