Then, somewhere over the Gulf, the new Super Heavy booster ran out of engines.

SpaceX launched the debut of its 408-foot Version 3 Starship from Starbase, Texas, on May 22, 2026 — the 12th flight test of the program and the first to fly the company’s long-promised, deep-space-capable variant. The upper stage soared. The booster did not. Five days later, the FAA dropped the hammer.

The agency formally classified Flight 12 a “mishap” and grounded the world’s most powerful launcher pending a SpaceX-led investigation it must approve. The timing could hardly be worse: SpaceX is barreling toward what would be the largest IPO in history, and Starship is the whole pitch.

Quick Facts

Flight: Starship Flight 12, the debut of the V3 vehicle, launched May 22, 2026 from Starbase, Texas.

Failure: Super Heavy Booster 19 lost an engine ~1:42 into ascent, then failed to relight enough engines for boostback — ending in a hard splashdown in the Gulf.

Upper stage: Ship S39 also lost a Raptor but completed its trajectory, deploying 20 dummy Starlinks plus two camera satellites.

FAA action: Declared a mishap on May 27; SpaceX cannot fly Flight 13 until the investigation closes and the license is updated.

What V3 changes: All-new Raptor 3 engines, a taller airframe, ~100-ton payload class, and orbital refueling and Starlink-deploy hardware.

Stakes: SpaceX’s ~$1.75 trillion IPO, targeted for mid-June, leans heavily on Starship reaching orbit.

Two Minutes of Glory, Then the Engines Quit

The launch itself was a spectacle. Lifting off from Starbase’s new Pad 2 — purpose-built for the larger V3 — the rocket cleared max-q and powered uphill on 33 third-generation Raptor engines. For SpaceX, simply getting the new hardware off the pad cleanly was a win.

Then the cracks showed. About a minute and 42 seconds in, one of the 33 Raptors on Booster 19 shut down. After the planned staggered shutdown and stage separation, the booster flipped for its boostback burn — and several engines that were supposed to relight simply didn’t.

“It looks like we just had an early boostback shutdown,” SpaceX’s Dan Huot said on the live broadcast as the telemetry graphics flickered red. The partial burn ended early, and Super Heavy — unable to slow itself for a controlled return — came down hard in the Gulf of America.

SpaceX’s official replay of the Starship V3 Flight 12 launch from Starbase on May 22, 2026.

The Ship That Kept Flying

If the booster wrote the bad headlines, the upper stage quietly stole the show. Ship S39 lit its six Raptors, lost one of them, and still reached its intended suborbital path by burning the survivors longer than planned.

It then did what no previous Starship had managed so cleanly: it deployed a full batch of 20 dummy Starlink satellites through a new PEZ-style dispenser. Two of those were real Starlinks rigged with cameras to film the vehicle in space — the first time SpaceX has scanned its own ship in orbit.

The ship survived a punishing reentry, flew a banking maneuver mimicking a future Starbase catch approach, and splashed down on target in the Indian Ocean off Western Australia before tipping over and exploding, as Starships tend to do.

The FAA Slams the Brakes

Initially, the regulatory picture looked survivable. The FAA acknowledged an “anomaly” over the Gulf and activated a debris response area — which pushed a handful of aircraft into holding patterns — but stopped short of demanding a formal probe.

That changed on May 27. “After a thorough assessment of the operation, the FAA has determined the May 22 SpaceX Starship Flight 12 launch resulted in a mishap,” the agency wrote, pinning it on Booster 19’s flyback. There were no reports of public injury or property damage.

The practical effect is a grounding. SpaceX must run the investigation, the FAA must approve the final report and any fixes, and the launch license must be updated before Flight 13 flies. The company moves fast — a Falcon 9 grounding in February lasted just four days — but the brand-new Raptor 3 engines have no flight history to lean on.

“SpaceX is thoroughly reliant on Starship becoming reliable and reusable in order to continue growing its Starlink service, which is by far the company’s biggest revenue generator and currently its only profitable business.”
Sean O’Kane — Senior Reporter, TechCrunch

What V3 Actually Changes

A NASA artist’s concept of the Starship HLS variant on the lunar surface. The V3 architecture is what NASA is counting on for Artemis. Credit: NASA.

Version 3 is not a tweak; it is close to a new rocket. It introduces all-new Raptor 3 engines, a taller airframe, and a payload class Musk pegs near 100 metric tons. The Raptor 3, SpaceX says, produces more than double the sea-level thrust of NASA’s SLS core.

Crucially, V3 is the first variant SpaceX calls deep-space capable, with hardware for orbital propellant transfer — the linchpin of NASA’s Artemis plan to put astronauts back on the Moon. It is also the first Starship designed to actually start deploying operational, next-generation Starlinks to orbit.

That ambition is exactly why a debut failure stings. Every one of those upgrades — new engines, new dispenser, new pad — is now data for an investigation rather than a victory lap.

A Grounding With an IPO on the Clock

Here is where the rocket meets the prospectus. SpaceX filed its S-1 with the SEC on May 20, targeting a Nasdaq debut around mid-June under the ticker SPCX at a valuation reported near $1.75 trillion — which would dethrone Saudi Aramco as the biggest IPO ever.

The catch: that valuation rests on Starlink, which accounts for roughly 70 percent of SpaceX’s revenue, and Starlink’s growth rests on Starship reaching orbit to haul the next-gen satellites. SpaceX’s own filing warns that anomalies and “unforeseen technical challenges” in Starship could delay its other business objectives.

Flight 12 delivered both halves of that story at once: a ship that proved the orbital deployment concept, and a booster that proved the program is not done failing. Investors get to weigh a partly successful debut against a fresh FAA grounding, in the same week they decide whether to buy in.

What Happens Next

SpaceX already has Flight 13 hardware in flow — Ship 40 and Booster 20 — but the combination of a failed boostback and an engine-out on the ship makes an orbital attempt or a tower catch unlikely on the very next flight. Expect another booster splashdown attempt instead.

The single biggest unknown is the one nobody can yet answer: why those Raptor 3 engines refused to relight for boostback. SpaceX has not published a root cause, and the FAA-overseen investigation exists precisely to find it. Until then, the most powerful rocket ever built stays parked.

For a company about to ask the public for the richest IPO in history, “mostly successful” is a phrase that cuts both ways.

Sources: Spaceflight Now (Will Robinson-Smith); Space.com (Mike Wall); TechCrunch (Sean O’Kane); FLYING Magazine (Jack Daleo); FAA statement; SpaceX mission update.

For roughly two minutes, the biggest rocket humanity has ever flown behaved exactly as Elon Musk promised it would.
